Who Can Use the Program?

* Medicaid-eligible individuals
Persons eligible for Home Health Care Services, personal care services, or private duty nursing
* Those who require some or total assistance with one or more home care, personal care, or skilled nursing services
* Individuals who have a stable medical condition
* Those who are able and willing to participate in the selection, supervision, evaluation, and termination of qualified service providers
